Job summary
Southern Health Foundation Trust has an exciting opportunity for an experienced Diabetes Nurse to join The Willow Group, a forward thinking and innovative GP group providing primary care services for over 50, 000 patients over 5 local sites in Gosport.
We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously.
The post holder will be an experienced nurse, acting within their professional boundaries to deliver evidence-based practice for patients with long-term conditions. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Main duties of the job
To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. The focus of the role is both the delivery of evidence-based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team and the wider community teams to meet the needs of patients, support the delivery of policy and procedures and provide leadership where required.
