Job summary

Do you have a passion for Falls Prevention, the ability to work unsupervised, are able to prioritise workload, have excellent communication and IT skills as well as the interest and enthusiasm to deliver training in falls awareness and prevention?

We are seeking to recruit either a registered Nurse, Physio or Occupational Therapist 15 hours a week (0.4 WTE) to work in the Falls Prevention Team.

The post will predominantly cover the Southeast -- Gosport, Havant, Fareham and Petersfield, but there will be a requirement to travel to other parts of the Trust for team meetings and training.

Under the supervision and encouragement of the Service Lead and Falls Coordinator: your role will predominantly include community work visiting patients in their own home, triage, equipment, facilitating Slip Trips and Falls training, health promotion talks, support to teams with falls prevention documentation, audits, and incident analysis data collection.

You will receive a comprehensive induction program and will be mentored and guided through in-house training to become a Falls Prevention Coordinator.

The applicants must be able to travel across the trust, so driving licence and access to a car is essential.

Main duties of the job

The post is for a qualified Nurse, Physio or Occupational Therapist who wants to develop their specialist clinical knowledge and leadership in falls prevention and management.

They will work with the Specialist Falls Prevention Team to implement falls and bone health strategy across Southern Health NHS Foundation Trust (SHFT), to ward staff, community staff and nursing homes Hampshire, liaising with health and social care professionals, neighbouring secondary care Trusts, local authorities, statutory bodies and voluntary agencies..

Undertake specialist nursing/therapeutic assessments (appropriate to clinical qualification and profession) of patients at risk of falls with diverse and voluntary agencies to develop prevention strategies.

Provide team leadership and support.

Provide training and support to the falls link practitioners.
