Job summary

Alton Community Hospital is a 40 bedded community hospital providing high-quality rehabilitation and care for local patients. We are currently seeking an enthusiastic and forward-thinking Trainee Advanced Clinical Practitioner to join our team.

The primary focus of the role is to work as an advanced, autonomous practitioner providing patient-centered clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. Working with patients and their families to provide a home first approach to care. Alongside rehabilitation the hospital also provides personalised palliative and end of life care to local patients.

As an advanced practitioner you will assess and treat people with a range of conditions and implement a person-centered plan (including rehabilitation) to enable that person to recover, and re-engage with the important aspects of their life.

The role allows you to implement a breadth of interventions, while having the support of a consultant practitioner and the multi-disciplinary staff on the wards. This rewarding role allows you the time and scope to explore what's important for each person and work with them to improve their health and well-being.

The team is looking to expand to cover a 7-day service. Part time/flexible working requests considered.

Main duties of the job

The post holder will be a trainee, working under supervision, to develop a high degree of autonomy and complex decision making. This will become underpinned by a master's level award, completed during the training programme, that encompasses the four pillars of clinical practice, leadership and management, education and research with demonstration of core capabilities and area specific clinical competence.

They will manage, under supervision, clinical care in partnership with individuals, families and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.

They will work towards exercising autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for decisions made.

They will work alongside, leading and clinically supervising the registered and unregistered staff members within the team.

Essential

  • Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC.
  • Professional UK registration e.g. NMC / HCPC
  • Meet the criteria for entry on to an accredited ACP Masters programme,
  • OR
  • Registered on and currently completing an accredited ACP programme,
  • OR in the process of evidencing equivalence of Advanced Level practice capabilities via the portfolio route.
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and report
