Job summary

We are seeking either newly qualified and or experienced nurses to provide complex care and treatment to adults living in the area who have long-term or complex health needs.

Main duties of the job
You will work in a multi-disciplinary way alongside a range of specialist services, including those in the community, social and voluntary services. The team work closely with GPs and developing these relationships for the benefit of our patients is a key part of the role.
You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that holistic assessment and care plans are recorded in line with the Trusts' standards. The role will also develop you to managing the co-ordination of a shift and supporting and line managing junior staff.
