An exciting opportunity has arisen to provide Podiatry services to a high standard within the Gosport Primary Care Network. Working in the Gosport PCN allows for a new and innovative working arrangement, based on population health management. This is an exciting opportunity to develop a working model that will benefit the PCN.
The PCN is committed and passionate about promoting health, well-being and independence. We are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care. The PCN works closely with health and social care professionals across all disciplines. It strives to empower patients, families and carers in both a home and care home environment.
This post requires a podiatrist who is highly specialised and has considerable knowledge, clinical skills and experience at a senior level, in the management of patients with lower limb pathology e.g. musculoskeletal, dermatological, neurological and vascular.
The post holder will provide effective and efficient delivery of care for foot and lower limb problems, using precision clinical skills and providing highly developed diagnostic services. The podiatrist can work autonomously as part of a motivated PCN team providing a high standard of clinical care in a variety of settings.
Gosport PCN has a vision to develop a frailty team to support frail patients in the locality. Due to this vision a special interest in frailty would be a great benefit to this role.
Main duties of the job
As a Primary Care Network Podiatrist, the post holder will work as part of a multidisciplinary team across the practices in the Gosport Primary Care Network (PCN), with the PCNs GPs, other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. Primary Care Networks aim to bring care closer to patients in the community.
