Job summary
The post holder is a highly skilled, senior practitioner able to operate at an advanced level of practice, with expert knowledge in the management of patients' dietary issues and conditions. They will work as an integral part of the MDT within the Primary Care Network.
The dietitian will provide interventions for the patient from initial clinical assessment, diagnosis, treatment and evaluation of their care within set cohorts. They will demonstrate safe, clinical decision-making and expert care for patients within general practice. These include a complex and wide range of diet related conditions across the lifespan. They will work collaboratively with the general practice multi-disciplinary team to meet the needs of patients, supporting the delivery of policy and procedures. They will provide dietetic leadership as required.
Gosport PCN has a vision to develop a frailty team to support frail patients in the locality. Due to this vision a special interest in frailty would be a great benefit to this role.
Main duties of the job
As a Primary Care Network Dietitian, the post holder will work as part of a multidisciplinary team across the practices in the Gosport Primary Care Network (PCN), with the PCNs GPs, other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. Primary Care Networks aim to bring care closer to patients in the community. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
