An exciting opportunity has arisen for a dynamic candidate to join our Frailty hub as a Parkinson's Clinical specialist Nurse at Southport and Ormskirk District Hospital.
You will receive a generous and comprehensive training package from Parkinson's UK - Ideally the candidate will already be working as Parkinson's disease Clinical Nurse specialist Band 7 , however we will consider candidates with a keen interest who wished to develop into this role at a Band 6 with a view to Band 7 on completion of training - this will include a Non Medical Prescribing qualification. (Annex21 will be applied in this case).
Main duties of the job
- To be responsible for undertaking specialist Nursing assessments, planning, implementing, and evaluating of patients within the Parkinson's disease (PD) caseload as delegated by the clinical lead.
- To manage a case load of patients using evidence-based practice and individual centred goals to assess plan, implement and evaluate interventions.
- Responding to service users needs for specific interventions within the scope of practice that will aim to enhance patient experiences, reduce length of stays, reduce waits for patients with PD to see a specialist and unplanned admission avoidance.
- To work as part of a multi-disciplinary team ensuring the best interests of the individual is met with the aim to prevent unplanned admission and readmission into hospital.
- To provide expert support, advice and education to raise awareness of the needs of people and their carers with this chronic progressive condition.
- To provide education and raise awareness amongst the wider MDT, and to participate in a training programme.
