TOlder people CMHT service for Southwest York
Locality as well as
Selby has an over 65 population of about 22000.
The CMHT provides Monday
Friday 9am - 5pm assessment and treatment services whichinclude the provision of care for older people with
functional mental health problems as wellas complex organic care for patients in their own
homes. The entire southwest team receives about 400
referrals each year.The approximately the West team
receives around 50% of the southwest team referral.
The post holder is expected
to see approximately 6 to 8 new CMHT patients a month.The team implements
purposeful and productive community service model.The postholder is expected
see approximately 8 to 10 review CMHT patients a month.This post has 1.5 PAs of
Memory clinic work inbuilt to make it a fulltime post.The post holder is expected
to see around 9 new memory patients in every 2 weeks period.
The memory initial
assessment and cognitive testing are completed by Memory Nurses.The post holder is not
required review previously seen memory service patients.
Memory Monitoring nurses
review patients who have previously seen the consultants. At the time of writing this
there are no CTO or section 37/41 patients.Usually there is no more
than 1 or 2 patients a year per consultant under MHA.
Main duties of the job
The
post holder will be responsible for providing a community based psychiatric service
to older people in the South-West Community Hub, working in and providing
clinical leadership to the West older peoples team (7.5PAs- DCC & 2.5 PAs - SPA).
To work with fellow consultants, clinical colleagues and service
managers to provide an assessment and treatment service for older people with
mental health problems.
