Job summary
The post holder will, with their consultant
colleague, see all presentations, requiring the input of a psychiatrist arising
from the secondary schools and their feeder primary schools within the Selby
sector. The consultant would expect to hold a caseload of around 70 based on
current consultant caseload and would be asked to see approximately 2 new
assessment per week. The service supports a model whereby all cases are co
worked with other members of the MDT, care coordinator and lead professional
role usually held by a non-psychiatrist, except where there is a formal mutual
agreement that sole consultant input is required with other members of the
multidisciplinary team.The Consultant joining the team will be
expected to work in accordance with GMC guidance for working in multiagency and
multidisciplinary teams. The Consultant will be expected to work flexibly.
The post will follow a 5.5 PA (Direct
Clinical Care): 2.5 PA (Supporting
Professional Activities) job plan. This
will allow the post holder to develop a protected day each week to pursue
additional interests such as research, medical education or leadership
opportunities and a special interest session, for example in medical education,
research or psychotherapy.
Main duties of the job
The
Consultant will have responsibility- together with the other Consultant in the
team- for the provision of psychiatric services to children and young people
living in the Selby area.
This
provision will include:
Clinics at Worsley
Court, with very occasional home visits where clinically indicated
Liaison with
Paediatricians based at York Hospital and other nearby hospitals as and when
appropriate.
Advice,
consultation and supervision to the team.
Involvement
in more complex cases using a co-working model, often in partnership with other
agencies.
Urgent work
including section 12 work, the post holder would be expected to have or be able
to hold within 3 months of commencing in post AC and section 12 approval, in
the meantime there will be emergency cover provided by colleagues from York.

The CAMHS directorate in North Yorkshire and York
locality of TEWV is comprised of the two York Community CAMHS teams based at Orca
House, the Selby team based at Worsley Court the Harrogate Team based at Dragon
Parade, the Scarborough Team based at Lake House and the Northallerton Team
based at North Moor House with a peripheral clinic in Colburn.
This post will provide input into the Selby part of
the North Yorkshire and York service. Specifically, this service is
commissioned by the Humber and North Yorkshire Health and Care Partnership .
It has a Single Point of Access service staffed by
an experienced staff team who triage and provide a telephone consultation
service to families and referrers. The SPA responds to all referrals and help
young people and their families with the process of navigating through the
options to find a service that meets their needs. The service works with a model
of brief intervention to empower young people and their families in achieving
good mental health and well-being. Staff job plans are designed to deliver
services described in the TEWV Clinical Pathways, in order to provide high
qualy evidence-based care.

Local
working arrangements
The Team is based at Worsley Court in Selby. It serves the Selby
District of North Yorkshire. It serves Selby and the surrounding
area, comprising the southern part of North Yorkshire. The population is around
83000 (2011 census). There are relatively high health inequalities relative to
the rest of North Yorkshire and pockets of deprivation in Selby North and South
wards where 1/3 children live in poverty.
The
team caseload is around 550.
The
Selby team has until recently been part of York CAMHS and has undergone
considerable reconfiguration in the few
years to establish itself as a separate service operating on an I-THRIVE model
to ensure continuity of care for service users.
A CAMHS crisis and home treatment team works alongside the
generic CAMHS as part of the Trustwide Alternatives to Tier 4 National Pilot
project. This provides a nurse led intervention 24 hours a day.
All referrals (except those coming through the crisis team)
are dealt with through a Single Point of Access for the whole locality. This
provides a screening assessment of all referrals into North Yorkshire and York
CAMHS.
There is also a
Trailblazer mental health support team as a part of a national pilot. The
service works into identified secondary schools and further education settings
within the Selby area. The team works as part of the staff team of the
educational setting and has three functions:
Support
the educational setting in the development, implementation and review of their
whole-school approach for social and emotional mental health.
Provide
advice and consultation to teaching staff within the identified educational
settings.
Provide
direct (either individual or group) interventions to pupils within the
identified educational settings. This will be brief (6 10 sessions)
using CBT principles.
An Enhanced Eating Disorders Team supplements the work of
the generic Eating Disorder clinic. The service is delivered in 2 specified
areas as a hub and spoke model. York is
the hub services, with Scarborough and Selby as the spoke service; this mirrors
the Harrogate service which is a hub with Northallerton as the spoke services.
All 4 parts of the service deliver against a shared service specification as
per the Access and Waiting Times for Eating Disorders (2016).
Psychological consultation and limited direct work to young
people in the care system is provided by a separate team PIPA team across the
locality. There are close links and regular coworking with this team to support
the mental health needs of looked after children.
Young people presenting with first episode psychosis are
assessed jointly by CAMHS and the Early Intervention in Psychosis team, with
the latter providing care coordination and liaising closely with CAMHS
psychiatrists.
There is a regional Forensic CAMHS service which provides
consultation and some direct input with young people who present a forensic
risk.
There is also a 12 bed tier 4 CAMHS inpatient service, Mill
Lodge, in York which is managed by Leeds and York Partnership Foundation Trust.
There are close working relationships between the team at Mill Lodge and
