Job summary
An exciting opportunity has arisen to join the MHSOP Scarborough Community Team, the team support individuals and families within the community, you will work alongside a multi-disciplinary team to provide high quality care to those accessing the service. Within this role you will have a good knowledge of local services within the area, support individuals into the community. You will need to be able to communicate well with a wide range of individuals, adapting your communication style according to individual need in a responsive and caring manner.
We are looking for people who can work relatively independently but will be supported regularly both clinically and managerially, supervision and support with professional development.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
Main duties of the job
Band 3 Community Support Worker is an essential clinical role required to deliver care as delegated by registered clinicians, including group work. This CSW post will contribute to consistent provision delegated non-pharmacological intervention and monitoring of mental and physical health; also enhancing communication about patient progression and increased needs to ensure responsive, appropriate support from within the small multidisciplinary team. The CSW's have an essential role to play in supporting Care Home Connections work to ensure positive working relationships and effective joint working between services.
Their agile approach allows them to link with Duty and Key Workers, respond promptly with low-level advice and interventions to reduce carer stress, and escalate concerns for deteriorating patients to prevent care breakdown and support the team to deliver prompt care that can help to avoid unnecessary hospital admissions. They have extensive knowledge regarding community resources for patients, family and carers to support individuals to stay connected. CSWs also underpin the delivery of the team's Physical Health Clinics, which cover patients under both the Memory Service and CMHT. They provide timely physical interventions such as phlebotomy and ECGs, ensuring safe monitoring during medication changes and early detection of health concerns. This role is fundamental to the smooth operation of the MDT and the delivery of core clinical pathways.
