An exciting opportunity has arisen for a Community Mental Health Nurse to join our Memory Team in Redcar, based at reed Marsh House. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people with cognitive impairment, their families and carers, and with a specialist interest in delivering high quality, effective and place-based Memory Services. This is an exciting time to join our team and locality - this role is an important part of a quality improvement initiative, alongside the trust Dementia pathway refresh. We deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. This is essential to enable timely diagnosis of dementia and associated needs and conditions, which supports access to early treatment and support, decision making and aids optimal management and life outcomes for those in need.

Main duties of the job

The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.

In the role you will be responsible for making the initial contact to support service users and their families and carers in their understanding of the memory pathway. You will undertake a detailed history using a holistic memory assessment framework, with the initial collation of all clinically relevant individual service user information (clinical history) and involving families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.

There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues.
