Community Support Worker/Trainee

Job summary

An exciting opportunity has arisen for a support worker/trainee to join the assertive outreach and recovery team which supports patients in the community with their mental health recovery. The role involves working with people in their homes, the community clinics and also providing in reach to patients who are admitted to hospital to support their discharge home.

The post holder will work as part of the multi-disciplinary team delivering high quality patient focused care in a community setting.

The role will involve improving the lives of people with mental health issues and the impact of their condition through the delivery of excellent services to promote recovery and wellbeing. It will involve promoting patients independence and also offering some physical health interventions.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

Main duties of the job

The post holder will work in conjunction with the multi-disciplinary team to promote recovery for individuals with complex psychosis, poly substance, housing and complex social and emotional needs. Patients who are working alongside assertive outreach are often disadvantaged and marginalised from society.

We are looking for a post holder who has experience working within substance use services or with individuals experiencing substance use challenges, however this is not a requirement.

The successful candidates who do not have the required qualifications will be expected to enrol on the Healthcare Assistant Framework which will include the acquisition of the level 2 Diploma Health and Social Care, level 2 in Literacy and Numeracy or equivalent and achievement of competencies. You will have a maximum of 2 years to complete this.
