An exciting opportunity has arisen for a Registered Nursing Associate to join the Sedgefield North Treatment and intervention Team. The applicant must have completed the Nursing Associate foundation degree and be a registered Nurse Associate on the NMC register. The successful applicant will work as part of the busy community team offering a recovery focused pathway to Service Users and act as a keyworker for a defined caseload. You will support with both medicines management and psychological approaches in your delivery of interventions to meet individuals needs. You will work collaboratively with patients and their families in order to support with goal setting and meeting identified need.
Main duties of the job
The successful applicant will work as part of the busy integrated community mental health team offering a recovery focused interventions to service users. The applicant will manage a defined caseload acting as keyworker with supervision and support from the wider multi disciplinary team. This role will involve multiagency working and liaison with other services within Tees Esk and Wear Valley NHS Trust.
Following initial patient assessment completed by the Registered Nurse you will be responsible for implementing care packages, including providing information and advice to patients, clients andcarers. In collaboration with the Registered Nurse evaluate and adjust programmes of carein partnership with patients, clients and carers and the multidisciplinary team.You will promote health and provide and monitor care with all patients and carers. Delivering interventions to a high standard in line with the Trust governance framework.The role requires you to respond to enquiries from relatives and carers, dealing with concerns and actively and problem solve to improve the patients experience, escalating to the registerednurse as required.Accurately undertake risk assessments, using contemporary assessment tools to improve safety and quality of care alongside the registered nurse
