Job summary

An exciting opportunity has arisen for a competent, confident, and enthusiastic full time permanent Band 6 community psychiatric nurse to join our team. The Persistent Physical Symptoms service is based within adult mental health planned care services covering the County Durham and Darlington area.

The recently expanded multi-disciplinary team works with patients who experience persistent medically unexplained symptoms including functional neurological disorders, predominantly in community settings to deliver care and treatment to enable individuals to reach their recovery goals.

We are looking for a nurse who will be able to demonstrate a highly developed knowledge and understanding of, and experience working with a wide range of mental health conditions including those with sensory needs and can work flexibly to meet service requirement.

The post holder will provide evidence-based nursing assessment and interventions for patients with functional neurological disorders. They will be able to demonstrate a commitment to the assessment of patient need and the therapeutic use of intervention to support patients reach a meaningful recovery of ordinary lives, daily roles and occupations. They will have the vision, energy and inspiration to develop creative, imaginative solutions to delivering nursing intervention in this clinical specialty. They will contribute to the assessment and development of collaborative plans to manage risk as part of the MDT.

Main duties of the job

As a registered professional this is an ideal opportunity for someone who wants to develop their leadership and clinical skills in a community setting. As a core member of the team, you will also be responsible for delivering intervention in the community with people who have complex needs, ranging from mild to moderate level of need to severe and enduring level of need and complexity. You will ensure that the service users' recovery goals are identified and central to their Safety plans, promoting social inclusion, independence and well-being. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.

There will be a requirement to work autonomously and in partnership to develop new ideas and enhance pre-existing service improvement initiatives to progress the efficacy and quality of the service provided.

There are varied opportunities for training and development, with CPD needs being identified and monitored through appraisal. There is a requirement for the applicant to hold an up-to-date and credited non-medical prescribing qualification, or willingness to work towards this within an agreed timescale. There is also a requirement for the applicant to look at expansion and development of the service and ensuring the specialist nature of the provision is reviewed and improved systematically.
