This is an ideal opportunity for someone who wants to develop their leadership and clinical skills in a community setting within the Single Point of Access (SPA). You will demonstrate excellent assessment and analysis skills, interpersonal, communication and team working skills. You will need to be committed to working collaboratively with a wide range of statutory and third sector agencies.
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
Main duties of the job
The Senior Community Mental Health Nurse would be able to offer specialist mental health advice, support and education to the services within the SPA and be a key member of the Hospital at Home Team, complementing and supporting the work of the acute trust's urgent community response. The nurse will support discharges from the acute hospital where there is an identified mental health concern and will link in directly to services within TEWV to support the client
The nurse would be a visible practitioner within the SPA and be essentially involved in the triage of referrals where there is a mental health need. The nurse will be able to accept work on behalf of TEWV and liaise with the relevant teams. The clinician would also be able to take work from TEWV into the SPA and discuss for action.
The Senior Community Mental Health Nurse would use and build on existing relationships with services involved in SPA and attend meetings within SPA to represent mental health services at TEWV.
The Nurse would be able to provide training to services who are involved within the SPA andwould work with SPA to continuously review and evaluate the services offered.

The role will be primarily based at Billingham Health Centre, but will also be part of the wider Intensive Community Liaison Service (ICLS)
