An exciting opportunity has become available for a 12 month fixed term/secondment post within the team for a band 5 community nurse.
The ALD South Tees Community Team provides services across both Middlesbrough and Redcar and Cleveland based at Flatts Lane Centre and is made up of Nursing, Psychiatry, Psychology and AHP's (including Physiotherapy, Occupational Therapy, Speech and Language Therapy) staff.
The team work collectively and collaboratively with others to ensure a high standard of care is provided and are always willing to go above and beyond what is expected when required.
The teamwork in partnership with key partners to ensure people with a diagnosed learning disability and their families and carer and supported by delivering high quality clinical care.
The service runs 24 hours a day seven days a week with the expectation to cover 8-8 Monday through to Sunday and be part of an On-Call service 5pm -9am.
Nursing applicants should already hold NMC registration. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.
Main duties of the job
An exciting opportunity to join a highly experienced multi-disciplinary team within the Teesside locality working and supporting people with Learning Disabilities to achieve high quality of life and optimum health.
The successful candidate will hold and manage a case load, working collaboratively with service users, their families, carers and partner agencies.
The post holder will require excellent communication skills and be calm, confident and resilient in your approach, and will need to be able to form therapeutic relationships. The post holder should be flexible, adaptable and be able to travel independently during the course of their duties.
