Job summary

We have an exciting opportunity for a Band 6 Hospital Liaison Clinician to work within the Northallerton Acute Hospital Liaison Service; providing mental health in reach into Friarage Hospital, Friary Hospital and selected care homes within the Hambleton and Richmond area.

The AHLS Team work into the urgent treatment centres and the hospital wards, supporting adults with a range of mental health symptoms and safety needs. People referred to the team may have complex comorbid physical health and social needs.

You will be part of a dynamic, welcoming and professional Multi-disciplinary Team that provides assessment, guidance and support to the hospital sites and the Hospital at Home team.We will offer you training and supervision that ensures you have the knowledge and skills to enable you to effectively and safely assess and support service users presenting with complex mental health needs.

We are looking for a passionate, dedicated and motivated clinician who is excited about working with comorbid mental health and physical health needs. You need to have the ability to work flexibly, autonomously, and to have effective communication skills, as you will be liaising in a dynamic environment with a wide range of professional colleagues, partners and patient groups.

You will receive regular management and clinical supervision and appraisal, and will have access to development and training opportunities aligned with the team and service delivery model.

Main duties of the job

Daily core duties include:

  • Liaising with staff in the acute hospital, supporting them with the management of the mental health and well-being of patients who have been admitted for physical health care, but who require support with new or existing mental health need.
  • Responding to referrals asking for a mental state assessment for people presenting with an acute mental health problem in urgent treatment centre or on the hospital ward.
  • Being professionally accountable & responsible for giving specialist advice & support to facilitate the mental health care given to patients in the acute hospital.
  • Presenting the views of patients and carers to the clinical teams in the acute hospital to facilitate collaborative working between all parties.
  • Liaising with other professionals & agencies in the facilitation of a safe & effective plans of mental health care.
  • Liaising with other professionals and agencies in the development & facilitation of safe & effective discharge plans. This may also entail follow up appointments being delivered to selected care homes as part of our delirium pathway and/ or include one off assessments in conjunction with the hospital at home team to support admission avoidance.
  • Enhancing knowledge & skills in the field of mental ill health for staff employed in acute hospital and the Hospital at Home team through the provision of specialist advice & training.
