Job summary
An exciting opportunity for a full time Community Associate Practitioner within Middlesbrough Community Hub Team.
The role will involve working alongside the other health clinic team members under the supervision of senior practitioners and be responsible for working with service users and supporting them with their physical and mental health.
In addition you will be asked to support the clinical team in the delivery of interventions to enable the recovery of service users, under the supervision of senior clinicians.
The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into the position on Band 4.

Main duties of the job
The successful candidate will support service users with a wide range of mental health needs to monitor their physical health. This will include those who are prescribed anti-psychotic medications, ADHD medications, and lithium and those identified as having a Serious Mental Illness (SMI) prescribed other treatments not listed.
They will be asked to hold a caseload of individuals who attend these clinics.
in addition, the successful applicant will be asked to support the team in the delivery of interventions, enabling the recovery of service users.
