Job summary
We have an exciting opportunity for practitioners to be a part of developing and working up a Street Triage model working with Durham Constabulary.
The team will offer advice to the police to assist their decision making and ensure people have access to the correct support, ensuring they get the right help at the right time and the right care from the right person.
If it is identified that the person is in a mental health crisis, then the STT practitioner will be contacted by the police, so that the STT can conduct a full triage with the person, to determine the most relevant response, based on need and risk.
If the STT clinician cannot triage via the telephone, they will need to conduct a face to face triage or assessment and then devise a plan following this assessment which will consider the needs and safety of the person in crisis.
In this role you will work closely with police colleagues in the force control room, and support offices in the neighbour policing teams.
You will also work with other urgent / emergency care services in coordinating and effectively managing people detained under section 136.
18 months post qualifying experience including sign off of appropriate preceptorship or competency framework is essential.

As a member of the street triage team, you will network and develop relationships with urgent care, police, social services, voluntary services, to provide direction and the right care / support that is needed for individuals within County Durham and Darlington.
