Job summary

An exciting portfolio opportunity has arisen for a full-time experienced Physiotherapist (Band 6) to join our Mental Health Services for Older People (MHSOP) in Ryedale (based in Malton) - this post will work across both Inpatient & Community-based services.

The successful post-holder will work 3 days weekly at Springwood Inpatient Unit (Malton) & 2 days with the Ryedale MHSOP Community Team (Malton).

Springwood is a 14 bedded ward for patients living with Dementia who also have additional frailties, vulnerabilities & behaviours that challenge. Patients are transferred from assessment & treatment wards in North Yorkshire where it is identified that they require a longer period of treatment in hospital or may have had failed placements in care home environments. Our aim is to continue assessing patients & reviewing & updating treatment plans. Our focus is on positive behaviour support & the development of support plans that then follow patients through discharge into their new homes.

You will also be a valued member of our MDT in Ryedale MHSOP CMHT. This team has a genuine interest in collaborative working with older adults with functional and/or organic mental health problems including their carers and families. You will help the team to provide wellbeing & recovery focused care for individuals with complex & challenging mental health needs, including dementia, depression, anxiety & psychotic illnesses, within community-based settings.

Main duties of the job

The successful candidate will be an autonomous qualified Physiotherapist providing assessment, treatment planning & provision for older people with Organic & Functional mental health problems, who have complex physical and psychological needs. Often these patients present with multi-morbidities, including needs around frailty syndrome. You will assess using appropriate outcome measures and deliver physiotherapy specific interventions to address frailty.

You will work holistically within both the Inpatient & Community MDTs, supporting people throughout their admission & those living independently or in care settings. You will work closely with other professions in both settings, contributing to Huddles, ward report out meetings, weekly frailty meetings, admission & discharge planning discussions. In addition to managing patients needs whilst they are on the ward, you may support successful discharge into patients own homes, or offer a handover & education to carers, families or care home establishments where applicable.

Significant post qualifying experience in Physiotherapy including experience working with older people with complex needs is essential. Experience of working with patients with mental health needs is desirable.
