Job summary
A newly developing Darlington Gateway Team are seeking a Band 6 Mental Health Practitioner to join the transformation of the service. The Gateway Team provides front end work including assessment, depot management, one to one interventions, groups and much more. The successful practitioner caseload sizes will generally be under 15 patients which will be a mix of long term patients on depot injection/117/CTO and patients that are offered one to one intervention work for to 12 weeks. This post will offer you a vast array of skills within the team but also working closely with our agencies outside of TEWV, social care and others.
You will need to be enthusiastic and highly motivated with experience in assessments and delivery of care for patients in a community setting. You will have access to caseload/clinical/management supervision within the team and will work closely with other professionals in the multi-disciplinary team. As a Senior Practitioner you will provide supervision to others and lead the team in delivery of patient care.
The community services run a in house training programme every year to encourage all staff in self development, which in turn provides high standard of patient care.
Main duties of the job
To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To work as part of a community team which may be integrated, workingcollaboratively and in partnership with local authority services.
To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients' homes and other community settings.
To promote social inclusion, community access and participation through the provision of patient care.
To be committed to and involved in activities integral to the Trust's QualityImprovement System.
To promote at all times a positive image of people with mental ill health.
To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.
