Job summary

We have an exciting opportunity within MHSOP Stockton for 2 band 6 practitioners to joint our team.

We provide a range of interventions guided by both the functional and dementia care pathways, to support people at home and to reduce the need for mental health hospital admissions. Interventions are delivered on either an individual or group basis for people predominantly in the over 65 age range with severe or acute mental health conditions including; behavioural and psychological symptoms of dementia, severe and enduring mental illness, associated with significant disability, psychosis and affective disorders and any mental disorder where there is significant risk of self-harm or harm to others.

Applications are welcome from those with a Nursing, Social Work or Occupational Therapy Qualification and appropriate registration.

Main duties of the job

As a Senior Practitioner, you will offer supervision and guidance to newly qualified staff, students and support workers.

You will work primarily within an organic setting but may be required to work across organic and functional pathways completing assessments of need and develop collaborative care plans with patients, carers and families.

You will provide evidenced based interventions as required and support the management of behavioural and psychological symptoms of dementia.

The role will require you to liaise with Mental Health Inpatient wards, care homes, social services and primary care colleagues.

The successful applicant will receive regular appraisal, clinical and management supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.   • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.

  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
