An exciting opportunity has arisen for a Community Mental Health Practitioner to join our Community Mental Health Team based at Huntington House, Monks Cross, York to work with older people with mental health needs within the community setting and to contribute to service development.
We are a very empathetic and supportive team who prioritise wellbeing and developmental opportunities to progress skills and experience. We maintain good staff retention and have had staff members return to the team due to their positive experience of being in the team. We recently had a leadership walkabout and a peer review held within the Trust where we received positive feedback that our team has an excellent culture and prioritised staff wellbeing and provides compassionate care to the people we support. We are regarded as a high performing team who regularly achieve their targets.
Alongside this we creative with our resources and cultivate an enjoyable working environment and hold regular social events.
This is a generic post and is open to occupational therapists and social workers as well as nurses. We would also be willing to consider part-time applicants.
Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.
Main duties of the job
As a community practitioner you will have a genuine interest in collaborative working with older adults experiencing functional and/or organic mental health problems including their carers and families.
As a registered professional this is an ideal opportunity for someone who wants to develop the community clinical skills. As a core member of the team, you will also be responsible for your own caseload, providing assessments, care co-ordination and delivering intervention in the community with people who have complex needs ranging from mild to moderate level of need to severe and enduring level of need and complexity. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and the Advanced Practitioner. For newly qualified staff considering the role you will be support with preceptorship development and learning and development opportunities to meet your individual needs.
