An exciting opportunity has risen for a band 6 Senior Mental Health Nurses to join the Community Rehabilitation in Tees. Due to recent investment you will be joining us at an exciting time to develop the service to meet the requirements of the Community Mental Health Framework (CMHF).
We are looking for enthusiastic and dynamic individuals who are able to work with the wider MDT, voluntary sector and third sector organisations. You will work intensively with individuals in the community or Inpatient wards alongside Community Mental Health teams as a co-worker to strive for best outcomes for our service users and provide them with the best possible care to support their own recovery. The services operates across 7 days a week 9-8 with enhancements on a weekend.

Main duties of the job
Key skills and responsibilities will include working in rotation with other senior Practitioners within the team to carry out Rehabilitation assessments to determine the needs of the individual and how these can be met by the team. You will be responsible for having oversight of Rehabilitation bed availability and supporting patient flow from inpatient settings as well as working alongside community mental health teams in assessing service user needs for community rehabilitation interventions. This will guarantee continuity of care for service users transiting into and out of rehabilitation services. You will be responsible for providing coordinating and delivery an individualised support package that will enhance their skills and keep them sage and well in the community. Effectively liaising with referring agents will be crucial within your role. The role will involve producing robust treatment reports and recommendations in accordance with Trust and local policies.
