Job summary
We are looking for an experienced clinician to work into a newly established project team, as a transitions/discharge clinician, within the exciting Care Home, Intensive Community Liaison service and Crisis provision for MHSOP.
Looking to facilitate people going into or in a hospital setting back to care home/ home for residents of Durham ICB. The initiative will proactively engage care homes and their staff, families and carers and wider partners around initiatives and support . Developing greater integrated working, trusting relationships with the wider community, and recognising the need more than ever for joined up thinking and service provision with physical and mental health and with social care providers to reduce barriers and increase care within a person's residence.
Developing understanding of partner agencies and local initiatives to prevent admission into a hospital setting by providing safe, effective care directly to the person and their families / carers in their residence. The incentive would support greater understanding through education, training, and role modelling around delirium and behaviours that challenge with care providers and wider care / staffing groups, allowing oversight and support to navigate and link up with the services available.
Main duties of the job
Working in a small team who will focus on this stream of work but with support and guidance from the wider service line, with oversight from the project lead clinician, Team Manager and Service Manager for programme delivery. Working in collaboration with the transformation lead, ICB, Local authority and inpatient/community hospital setting to identify potential barriers, communication issues, lack of integration which could prolong a hospital admission.
Able to develop good relationships with other agencies and staff across service and provide insight and planning to actively promote care in the patients preferred residence. Able to asses needs and risk in complex situations and systems to ensure admission to an inpatient setting is the right option for the patient.

Able to offer clinical knowledge to ensure safe pathways of care
