This is an exciting opportunity to join the Nursing and Governance Directorate as a patient safety reviewer. This is an opportunity in the patient safety team to review serious incidents and learn from them and to take forward the learning to help improve services. Working with families and the services you will have keen interest in patient safety issues and be aware of the NHSE/I Patient Safety Strategy (2019) Patient Safety Incident Response Framework. Learning from deaths is a key aspect of this role and supporting the Trust in the review incidents. You will be given full training and have the opportunity to work Trust wide taking learning forward. We will welcome interest and applications from any health profession who have up to date registration in their field.
Main duties of the job
The Serious Incident Investigator will take the lead in investigating allocated serious incidents on behalf of the Trust. This will include investigating under the new national frame work of PSIRF (patient safety incident response framework)

The post holder will:
- Conduct thorough investigations into serious incidents, using appropriate methodologies in keeping with NHS frameworks and regulations.
- Work in line with the Duty of Candour principles, being open and transparent throughout the incident investigation process
- Ensure staff feel psychologically safe to raise patient safety issues, working within the principles of Just Culture
- Draft comprehensive but concise reports outlining the findings of investigations in conjunction with the appropriate clinical service(s), ensuring that reports are written in an unbiased way.
- Meet with and support patient/families/carers during the serious incident investigation process, where appropriate, alongside the Trust Family Liaison Officer
- Share the findings of serious incident investigations with relevant stakeholders
- Carry out Structure Judgement Reviews to support mortality review work as and when required.
- Support staff in the preparation of statements for serious incident investigations and inquests as necessary.
- Categorise and classify serious incidents and other incidents requiring investigation to assist in the identification of themes and trends.
- Help to develop a culture of continuous learning and improvement ensuring that outcomes from investigations are fed directly into Trust-wide improvements.
- Promote and drive forward the quality improvement and patient safety agenda across the Trust
Whilst working under the direction of the Head of Patient Safety the post holder will be expected to demonstrate initiative and innovation.
