Job summary
We have a vacant permanent Band 5 Learning Disability Community Nurse/ Care Co-ordinator post. The post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
The service runs 24 hours a day seven days a week with the expectation to cover 8-8 Monday through to Sunday and be part of an on call service 5pm -9am.
Main duties of the job
An exciting opportunity to join a highly experienced multi-disciplinary team within the Teesside locality working and supporting people with Learning Disabilities to achieve high quality of life and optimum health. The role is community based within Middlesbrough Redcar and Cleveland, based at Flatts Lane Centre Normanby .
The successful candidates will carry and manage a case load.
You will need excellent communication skills and be calm, confident and resilient in your approach. You will need to be able to form therapeutic relationships with people with learning disabilities their families and carers.
