Job summary

o As part of the Integrated Community Stroke Service (ICSS) this post is trust wide across allareas of the Stroke therapy service including the acute ward to provide an acute 7 dayservice.o Providing care to a diverse population, to deliver a high standard of physiotherapy care tostroke patients and their carers in the community.o To perform comprehensive physiotherapeutic assessment of patients with diversepresentations and complex physical and psychological conditions following a stroke.o To asses stroke patients to develop and deliver an individualised treatment programmespecific to individual needs.o To hold responsibility for own caseload and, working without direct supervision but togetherwith the other members of the MDT, deliver a comprehensive package of care to strokepatients and their carers.o Supervision takes the form of supervision sessions, in service training, peer support groups.o Access to advice and support from a more senior physiotherapist is available when required.o To undertake all aspects of clinical duties as an autonomous practitioner.o To contribute and be involved in service changes/developments through quality improvementprojects.o To provide cover for other physiotherapists in the service as required and to cover weekendworking on the acute ward.o Must have UK driving license and access to a car.

Main duties of the job

  • To work autonomously to complete advanced assessments and treatment of patients within the speciality who may have complex presentations and to determine Physiotherapy treatment indicated and to maintain records as an autonomous practitioner.o To act as a specialist resource to other professionals for the management of patients following a stroke.o To represent the Physiotherapy Service at the multi-disciplinary team meetings to ensure the delivery of a co-ordinated multi-disciplinary service and integrate Physiotherapy treatment intothe overall management programme.o On a daily basis to implement decisions on allocation of limited resources which directly affectlevel of physiotherapy intervention to specific patients and have to justify those decisions topatients, carers and others.o To undertake therapeutic manual handling in compliance with Trust manual handling policiesand local guidance at all times.o To deal sensitively with patients and carers who have high levels of anxiety and aggressionE.g. caused by loss of mobility and independence, cognitive impairment e.g. dementia orlearning difficulties.o To interact with patients through an interpreter when English is not the patient's firstlanguage.
