Reporting directly to the 18 Week Senior Administrator, and accountable to the 18 week Elective Booking and RTT Manager of the service, the post holder will have the ability to lead, facilitate and co-ordinate the patient's journey throughout their 18 week pathway within University Hospital Tees. The post holder will provide an efficient, effective and comprehensive service supporting the 18 week tracking team.
The applicant will need the confidence to be able to prioritise the needs of the service across all the specialties covered by 18 week RTT. They must show confidence and ability to be able to support the clinical teams in their decision on the correct outcome of patients pathways live in a clinical setting. They must have a good knowledge and an understanding of clinical pathways and show confidence in the training of clinical teams whose patients cover many clinical pathways. The post holder will be required to ensure that all data relating to the monitoring of the 18 week pathways for patients is collated and processed in accordance with the national and local timescales for submission of data, thus providing timely information to the wider trust teams. They must be able to read reports provided and be alert to any discrepancies that may affect the figures for the 18 week pathways.
Main duties of the job
To monitor and track patients pathways by assisting with data collection, for audit and monitoring purposes and showcasing the service and role requirements
To ensure daily the 18 week tracking information is clear and precise and is up to a high standard. Updating the information gathered, to enable the care groups to be able to review the delays in some clinical pathways.
Be aware of the waiting times for both outpatient and inpatient services. Highlighting any service whose performance is affecting the delay in closing patients pathways.
Deal with queries from service users in a manner that is consistent with legislation, policies and procedures whilst demonstrating understanding, empathy and discretion.
Taking accurate written messages and passing more complex queries to relevant staff members where appropriate.
Maintain and support electronic system update regarding patient's location and movements
Support the clinical teams to share accuracy and efficiency for the outcome of a patient journey.
Access patient details using the trust electronic systems
Ensure diagnostic tests and results are not delayed and access patient results to support the medical team's decision regarding patient care.
Be involved in trust initiatives for the RTT reduction for the North East and North Cumbria Integrated Care Board catchment.
