Job summary

Health Care Assistants (HCA)/ are integral to the delivery of the respiratory service supporting people with respiratory conditions such COPD and interventions such as home oxygen and home NIV.

The post holder will work as part of a team consisting of specialist Registered Nurses and Physiotherapists and will support an 8am until 8pm, 7 days a week community service providing support to patients in the community setting.

Main duties of the job

The post holder will provide effective and safe care for respiratory patients within their own home environment, deliver pulmonary rehabilitation and support community clinics. They will demonstrate conduct that promotes dignity and respect throughout their care delivery. The post holder will undertake duties in their respective area commensurate with their band.

Job description

Job responsibilities

  • To assist the COPD Team in the delivery of high quality patient care, through a range of clinical procedures and clinical observations. This will include health related education, , perform diagnostic and monitoring investigations i.e. oximetry, spirometry, shuttle test, venepuncture, capillary blood gas measurements. (This list is not exhaustive). Delivery of patient care will be supported by completion of the competency framework.
  • To be able to use a wide range of specialist equipment and to access the relevant training for all equipment used in delivery of the HCA. role.
  • To establish a partnership with patients and carers / relatives, acting as an advocate in order to support them in achieving their goals to enable them to achieve maximum independence and improve quality of life and health status. Perform and adhere to individualised plans of care to patients within the community setting receiving care as part of the enhanced care pathway as well as carrying out telephone reviews within agreed protocols as directed by the clinical team
  • Be aware of when to recognise and escalate abnormal recordings or signs of change, within designated parameters, in a patients condition to the registered professional.
  • Support the education of specific client groups i.e. inhaler technique, care of equipment.
  • Promote self management and referral to support services i.e. stop smoking service
  • To establish and maintain good relationships with patients, carers, external agencies, as well as other members of the team.
  • To take responsibility for ordering, storing and maintaining nursing supplies. This includes ensuring clients with non invasive ventilators are supplied with replacement accessories on a monthly/as required basis without direct supervision. To ensure respiratory clinical areas are adequately stocked and tidy.
  • Maintain a database for equipment, ensuring efficient return, cleaning and maintenance of products. Maintain equipment in a clean and safe condition. To ensure that all equipment used is maintained and fit for purpose and be aware of personal responsibility in achievement of competencies in relation to the safe use of medical devices, including reporting of any defect or loss.
  • To assist the clinical team with data entry and administration duties.
  • To attend and participate in team meetings contributing where appropriate to service development. To participate in clinical supervision
  • To provide evidence of accurate records of work undertaken, through contemporaneous accurate electronic and paper health care records, collating performance data as required by performance management team.
  • Retrieval of patient information either from the electronic healthcare record or GP to support clinical team in providing safe care in the Hospital at home service
  • To ensure time management within own shift to ensure that records are updated in a timely manner by prioritising own day to day work-load, responding appropriately to changing requirements
  • To take responsibility for assessing own performance, identifying learning needs and how to meet them through supported active participation in the annual PDR process; this includes attendance at mandatory training and training identified as appropriate to the role.
