Job summary
The Rapid Response Team (nursing) provide nursing & social care interventions within community settings, with the clear aim of both preventing hospital admission & expediting hospital discharge. Key interventions include delivery of medications by intravenous & other routes, clinical assessment of people in the community, provision of nursing & social care packages as crisis intervention & varied other nursing duties.
Main duties of the job
Rapid Response is an innovative scheme providing nursing care, treatment and social support for patients in their own home preventing unnecessary unwanted hospital admission and facilitating safe early discharge.
We are looking for Staff Nurses to provide skilled nursing care to patients who access this service. Candidates will be responsible for assessment, planning implementing and evaluating care needs and be committed to providing high standards of patient care.
Experience of Community nursing, working within an emergency assessment unit or rehabilitation would be an advantage.
The hours of work will be on a shift rota between 08.00 - 22.00hrs over seven days a week.
The ability to travel in the community is essential.
