Job summary

We have an opportunity to appoint to a band 4 Trusted Assessor that will work as part of the Integrated Discharge Team and will be expected to work as part of a multidisciplinary based team with the Stockton and Hartlepool Borough Councils. The role will predominantly support and facilitate the discharge of patients from hospital to community bed based facility and will require close liaison with patients, families, health and social care colleagues. The successful candidate will be expected to assess the needs of patients to determine the correct level of support post discharge with appropriate follow up which may also involve community working.

The successful candidate may be required to work between the Stockton and Hartlepool localities but will be predominantly based at UHNT.

We are looking for applicants who are dedicated yet flexible in their approach and possess the aptitude to adapt quickly to new situations.

Main duties of the job

To provide support within the Integrated Discharge Team to facilitate the Trusted Assessor pathway for patient's requiring bed based community support this may include:

  • Assisting in the rehabilitation of patients.
  • Review and assess the rehabilitation/on going needs of patients and direct on to the most suitable pathway.
  • Complete assessment on behalf of social care to assist in the facilitation of a safe discharge from hospital.
  • Contributing to the assessment of therapy needs as initiated by the Registered Therapist.
  • Carrying out therapeutic interventions and programmes of more complex patients under the guidance of the Therapy Team.
  • To assist with the assessment and provision of equipment.

  • Communicate and exchange condition related information with patients / carers.
  • To monitor the progress of patients receiving interventions and communicate changes back to the team.

  • Following assessment advice patients of appropriate adaptive techniques.
  • Prescribe appropriate equipment and walking aids to enable patients functional safety and mobility.
