Job summary

The post involves working within the Pain Management Service providing clinical support to the multi-disciplinary Pain Management Team, ensuring high standards and quality of care throughout. This will include specific responsibilities for the assessment of patient's chronic and acute pain including management advice, treatment, review and discharge of patients within the service.

To work effectively as a pain specialist nurse contributing to high quality care, facing and dealing as a team to the many challenges faced.

Main duties of the job

The post holder will work in a multi- disciplinary dealing with patients in hospital suffering from acute pain , acute on chronic pain and also patients suffering from chronic pain problems in the outpatient setting.

A new and exciting part of this post is reviewing those patients who are waiting for procedures and struggling with pain ," Waiting well pilot " and help manage their symptoms hence improving their journey .

Make an advanced pain assessment and treatment of patients referred to secondary care who have complex, acute/chronic or emergency pain control requirements.

Be involved in regular education sessions on pain assessment, analgesia stepladder, pain devices used post operatively and be a resource to the hospital for advice and support.

Use a psychosocial model to patient

Job description

Job responsibilities

The post holder will work in a multi- disciplinary setting communicating within the team and external services, for example drug and alcohol team , physiotherapists, pain psychology , pre assessment.

Attend monthly MDT s relating to complex patients and therefore requires proficient communication skills in order to communicate complex/sensitive patient information, sometimes where there mat be barriers to understanding.

Provide patient education on pain tools, i.e. TENS machine.

Provide a supportive role in helping patients and their carers to manage and live well with their pain.

Order necessary investigations and provide treatment and care both individually and /or as part of a team and when appropriate through referral to other agencies/specialities.

Provide specialist support and health education to patients to help them to manage their condition.

Provide specialised advice and counselling concerning the care or treatment of identified groups or categories of patients/clients through skilled communication.

Participate in nurse led daily ward rounds and pain consultant acute pain ward rounds ( x 2 weekly )

Review patients directly referred to Waiting Well program to manage their pain whilst waiting for procedure.

Be involved in opiate reduction clinic and medication optimization.

Participate in the implementation/ review of policies using research to underpin practice, audit and measure outcome.

Participate in the operational management and organisation of the Acute/ Chronic Pain service.

Promote service development, acting as a change agent with excellent knowledge of change management.

Take responsibility for personal development and education and the development of personal development plan.

Manage own caseload and formulate individualised management and treatment plans.

Lead by example, promote on going professional development, service enhancement, and maintain/improve standards through research, clinical supervision and reflective practice.

Participate in service audits, reflection and monthly MDT meetings to move the pain team forward.

Maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities.

Participate in review of own performance regularly and development of personal development plan at least every 12 months
