The team support people who require domiciliary therapy for respiratory failure and sleep apnoea.
Due to secondment, a part time 12-month fixed term / secondment as a specialist respiratory NIV/Sleep disordered breathing practitioner within the team has arisen. This would be a fantastic opportunity for either a very experienced Band 5/6 healthcare professional looking to further develop their respiratory knowledge and experience or a Band 7 practitioner looking for a new challenge.
As a specialist respiratory Sleep/NIV practitioner, you will work alongside an established team providing assessment and domiciliary CPAP and NIV to a wide variety of respiratory failure /sleep patients in an outpatient setting.
We are a supportive team, valuing team and individual professional development. You will be provided with inhouse training to enable you to develop within the role.

- To organise and manage own caseload
- To line manage associate practitioners
- Develop a learning culture and facilitate interdisciplinary learning across respiratory medicine
- Take responsibility for the day-to-day management of domiciliaryResp NIV therapy within resource limitations
- Is an advanced autonomous practitioner who undertakes a comprehensive assessment of all patients referred to the service. Includes assessments of medical, functional, social and mental status.
- Develops a care plan and undertakes and co-ordinates its implementation, monitoring and evaluation.
- Work within defined guidelines/protocols/evidence base, delivering evidence-based care to a variety of patients with respiratory conditions.
- Is highly competent to an advanced level in the use/application of specialised respiratory equipment, including non-invasive ventilation and continuous positive airway pressure therapy. This includes the education in the safe and effective use of the equipment. Provides expert advice on this equipment to other health professionals including doctors and other MDT members.
- Is proactively involved with the coordination and management of patients requiring investigation and subsequent treatment. Uses results from medical assessment tools such as multidisciplinary notes, lung function, sleep studies and capillary blood gases to help inform clinical decision-making. Facts and findings may be complex or confusing and sound skills of analysis and interpretation are required in order to make appropriate judgments.
- To identify suitable patients for domiciliary non-invasive ventilation and set up in a timely manner to reduce risk of hospital admission. Ensuring appropriate on-going follow up is arranged.
- Assess results of sleep study to assess patients suitability for CPAP therapy, and recommend other therapies and lifestyle changes as indicated.
- Has clinical expertise in the management and treatment of respiratory and sleep conditions and acts as a resource to staff, patients and carers.
- Actively participates in MDM and team meetings.
- Provides appropriate education, counselling and psychological support for patients and their carers.
- Observes confidentiality of information and maintain comprehensive, contemporaneous records in line with policies and guidance such as NMC Guidelines for records and record keeping (2018).
- Is wholly accountable for own practice, is guided by principles and broad occupational policies or regulations and takes every available opportunity to sustain and improve knowledge and professional competence: not directly supervised.
- Delivers holistic care to meet the needs of patients as prescribed in care plans aligned with Fundamentals of Care.
- Works collaboratively with the Consultant Chest Physicians within the multidisciplinary team, and accurately reports findings following clinical assessment.
- Actively seeks opportunities to address health promotion for the patient group, e.g. smoking cessation and other health lifestyle activities.
- Provides highly specialised advice, support, education and counselling as appropriate to patients and carers for the management of their respiratory conditions.
- Formulates goals/clinical objectives jointly with patient/carers/relatives and other health care professionals and continually re-assess patients to evaluate progress. To alter and re-assess treatment programmes if required.
