Job summary

An exciting opportunity has arisen for a dynamic and highly motivated Nurse Consultant to clinical lead the Palliative Care Coordination Hub for SHC. This role and the team are part of a unique partnership between primary care, community services, social care, the voluntary sector, ESTH and St. Raphael's Hospice.

As Nurse Consultant for palliative care in the Palliative Care Coordination Hub (PCCH), the role will centre around the four pillars of advanced practice; notably clinical practice, leadership and management, education and research. The role will involve approximately 80% clinical practice and leadership/management with the remainder of the time spent on education and research.

Main duties of the job

The Nurse Consultant will be expected to undertake clinical practice at a specialist advanced level (including non-medical prescribing skills). The post holder will also be required to have excellent leadership, collaboration and communication skills to ensure an efficient and effective patient centred service.

The clinical lead role within the PCCH will be instrumental in supporting the delivery of:

  • Strategic vision and delivery of transformational joined up end of life care across Sutton, including support for families/carers
  • Clinical leadership/management of the PCCH team, working with partner organisations to deliver the coordinated response required from this team
  • In conjunction with the operational lead, support the development of the PCCH ensuring that the Hub develops strong clinical pathways and integrated working between all partner organisations across the Sutton Place
  • Provide advanced level, expert clinical practice in palliative care for patients wishing to remain in their own homes and provide support to professionals across Sutton
  • Develop and implement palliative care education/updates ensuring that all community teams are supported to develop their learning
  • Work with the Palliative Care Nurse Consultant Network across SW London for Sutton to ensure practice is stepped in high quality, evidence-based practice, tools disseminated and the Sutton community professionals practice is up to date
  • Lead for Nurse Prescribing across Sutton Health and Care (SHC).

Service Delivery

Working with the existing SHC Single Point of Access, the Nurse Consultant and team will provide hands on coordination and care delivery for end of life care. Critically, the Nurse Consultant and team will also support the early identification of those who need the service and meet the criteria and provide proactive case management and advance care planning with the aim of preventing unnecessary admissions and facilitating timely discharge from hospital after an admission.

Clinical Leadership

  • Expert clinical leadership to self and team members, providing sound leadership to the team and expert specialist palliative care support and advice to the team and SHC nursing professionals to inform them of the management of patients on the respective caseloads.
  • Oversight for clinical governance of the service and staff.
  • To undertake hands on clinical practice at a specialist advanced level (including non-medical prescribing skills) and provide clinical advice and care delivery for complex palliative care and end of life care to the team, professionals and others as requested.
  • To act as a clinical role model across SHC and external to SHC providing credible support and advice
  • To lead on standard setting, clinical audit and benchmarking in order to ensure the delivery of effective, evidence-based care.
  • To work with colleagues, patients and the voluntary sector across the Sutton system to develop clinical care pathways and promote a coordinated approach to delivery care that is dynamic, efficient, innovative and continuously improved in its relevance and responsiveness to need.
  • To lead/support on the development of initiatives to reduce and avoid unnecessary hospital admissions and reduce length of stay in hospital for patients identified as end of life.
  • To ensure that every patient referred onto the service has a Urgent Care record and advance care planning document completed, working closely with the patient and respective Sutton GPs to agree the plans sign off
  • Coordinate and develop a rapid response service to complement the existing SPC rapid response provided by the Hospice and generalist urgent response provided by the community nurses. This will be for patients needing palliative and end of life care, where professionals and/or the patient needs urgent changes to their management plan or professional need specialist support
  • Participate in the PCN, Community Response Team (CRT), Multidisciplinary Team (MDT) and virtual ward meetings when patients discussed are within the care setting of the PCC Hub
  • Lead on the development of robust patient and carer feedback on the efficacy of the service and how the service could improve
  • Ensure that arrangements are in place for out of hours (OOH) provision of palliative care medications, supported by the OOH GP including ensuring that arrangements are reviewed and communicated to relevant end of life care system providers
  • Ensure that accurate, contemporaneous records are kept that facilitates safety and continuity of care
  • Facilitate the use of information technology and ensure that relevant and up to date information on clinical and service developments are provided to agreed timescales
  • Participate in investigating complaints and clinical incidents within SHC Policy, procedures and timescales, making recommendations about changes to practice and service and set action plans to ensure that identified weaknesses are resolved
  • Assist in ensuring that clinical risk is assessed and managed in line with ESTH standards and to ensure that risk and adult safeguarding is appropriately managed within teams to ensure patient and staff safety and that adverse incidents are reported and investigated with an open reporting and learning culture being fostered
  • Regularly participate in clinical supervision and act as the lead clinical supervisor of junior staff
  • Take the lead for defined clinical projects at a corporate level as required and at a level appropriate to the post.

Managerial Leadership

  • Lead the team within the PCCH; this includes regular appraisals, 1:1s and performance management where necessary
  • Support in conjunction with the Operational Lead performance reporting and reports on the development of the service, as well as anything else requested by the corporate team for the Board and monthly assurance meetings.
  • Agree with the team members, clinical performance objectives to reflect both Trust and national strategies and priorities.
  • Ensure that absence levels for direct reports are monitored and managed
  • Ensure that individuals within the team are aware of and work within the Trust policies and procedures
