Role Summary

The role of Community Nursing Operational/Clinical Lead is fundamental to the success of Community Nursing Teams. The post holder will be able to demonstrate the competencies required to perform at this level and role model the Trust's core values as well as exhibit the behaviours required as part of those values.

The Operational/Clinical Lead will have the clinical expertise in Community Nursing to direct, support and develop staff in their clinical roles and will hold the Advance History Taking and Independent Prescribing Qualification.

Promoting a service which prevents patients from having inappropriate or unscheduled hospital admissions and helps facilitate hospital discharge. Implementing evidence-based practice to ensure best patient care.

Works alongside the Senior Nurses to ensure staff are competent in their role and new staff and inducted and supported to reach competency required for their role.

The Operational/Clinical Lead will also be responsible for all aspects of the leadership and development of teams of nurses. They will ensure high quality evidence-based care is delivered to patients, their families and carers in the Community. The Operational/Clinical Lead will be proactive in promoting a positive image of Community Nursing,

Main duties of the job

Ensure care in their locality is based on sound assessment and care planning

Ensure that appropriate Statutory and mandatory training has been undertaken by all team members.

Responsible for the retention, recruitment, induction, personal and professional development of staff.

Monitor clinical supervision, induction, preceptorship and development of staff.

Exercise leadership and judgment in the management of a Community Nursing Team ensuring good clinical practice and maintain high standards of care, review staff practice at regular intervals.

Ensure the service is meeting business and performance requirements of the Organisation in-line with service specification.

Investigate complaints and incidents, including Serious Incidents requiring investigation, safeguarding and clinical issues.

Identify the Root Causes, make recommendations for improvement and disseminate learning to staff, adding areas of concern to the risk register

Strengthen clinical leadership among Band 6 and 7 staff

Collaborate with the learning and development team to build a competency-based workforce that is trained, enabled and supported to deliver best practice care.

When required to work clinically alongside teams to ensure standardisation of practice embedding evidence-based practice.

Effectively communicate at all levels of the organisation; to a variety of health and social care professionals working in primary, community and secondary care, out of hours providers, voluntary organisations and patients and carers.

Provide high quality written reports and any other written documentation as necessary. To enter patient data on the EMIS system in a timely manner using where necessary the Team Planner, Daily Diary and Progress notes.

Work closely with the organisation, integrated teams, key stakeholders, and partners. They will promote a service which works within local and national directives.

Manage day to day capacity and operational planning, including rota management on behalf of the teams.

Ensure the service complies with all relevant policies and procedures that relate to GESH.

Participate in the Community out of hours on call rota

Contribute to the business planning process.

Clinical:

This role will consist of a minimum of 20% clinical patient facing activity.

Maintain defined competencies in core areas, as identified in the Clinical Lead competency framework.

Confidently and competently assess and provide interventions to patients that require urgent intervention to avoid hospital admission e.g., End of Life, blocked catheters.

Support teams to coordinate and prioritise the management of clinical caseloads with reference to patients with highly complex care needs.

Provide clinical support, leadership, and guidance to ensure that evidence-based care and clinical practice are maintained in the community nursing team.

Work in collaboration with the Primary Care Team and statutory and voluntary agencies to provide care for patients and their families and informal carers in their own home, within given resources.
