The Rehabilitation Support Worker role will function as part of the exciting Recovery At Home (RAH) Service by providing care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks, following a care plan and will report to the Daily RAH Co-ordinator and/or Senior on duty. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care.
ESSENTIAL: Full Uk driving License and access to a vehicle that is insured for work purposes.
Main duties of the job

Deliver clinical and therapeutic care per patient care plans.

Support assessment, planning, and delivery of care under supervision.

Identify need for referrals and follow through appropriately.

Provide handovers to team leads and the wider MDT.

Act as link worker in areas like tissue viability, infection control, and manual handling.

Promote health education per local and national policies.
-
Support patients' independence and daily living activities.
-
Assist with personal hygiene, grooming, and dressing while respecting dignity, culture, and preferences.

Address physical, psychological, social, cultural, and spiritual needs of patients, including end-of-life care.

Use risk assessment tools to ensure safety for patients and staff.

Perform clinical skills and observations as per competencies, including vitals, blood glucose, venepuncture, ECGs, PEG feeds, medications, and personal care.

Report adverse signs to team lead/MDT.

Maintain safe, prepared clinical environments.

Use IT systems and participate in data collection.

Provide evidence-based care and rehabilitation.

Communicate effectively with patients, relatives, and MDT members.

Maintain clear, accurate, and compliant documentation.

Uphold professionalism, confidentiality, and patient advocacy.

The service provides a 7 day / week service from 8am to 8pm and staff are required to work flexibly over the full week. We are open to conversations about flexibility with shift patterns.
Full time shift pattern example: 13 shifts of 8am to 8pm over a 4 week roster period including some weekend days.
Part time example shifts patterns could include 8am to 3pm, 8am to 12pm or 4pm to 8pm.
