Heart Failure Specialist Nurse

Job summary

Sutton Health and Care are firmly committed to providing the best possible care for residents across Sutton including those with long term conditions, such as heart failure and as such we have an exciting opportunity to recruit to a Band 7 heart failure specialist nurse for Sutton's heart failure- cardiology Team.

This post holder will be responsible for managing a caseload of patients with heart failure, both newly diagnosed and following their discharge from hospital. This will involve liaising with other members of the heart failure team including hospital consultants and GPs along the cardiac pathway from hospital to home, secondary to primary care to deliver evidence-based care to patients with heart failure.

The post holder will have the opportunity to work autonomously, but in close contact with, a number of partners in primary, secondary and social care as well as voluntary agencies.

As part of the new NHS plan the agenda for preventive working and in particularly with a focus on long term conditions, is truly exciting and the post holder will therefore be involved in the transformative neighbourhood work of the cardiology pathway from hospital to home.

We are looking to recruit a full-time permanent nurse to join our friendly, established, Cardiology team.

You will be an experienced nurse, who enjoys a challenge, and who has good communication skills, excellent clinical skills and is able to work with a high level of autonomy.

Main duties of the job

The post holder will:be responsible for managing a caseload of patients with heart failure,both newly diagnosed and following discharge from hospitalliaise with other members of the heart failure management team inprimary and secondary care to deliver evidence-based care to patientswith heart failureco-ordinate an education programme for community healthprofessionals and will also be a resource for advicework collaboratively within multi-disciplinary teams and in partnershipwith other services to provide seamless carework as part of the team and contribute to its success by being aneffective role model and supervising other staff i.e. nurses, healthcareassistants/support workers and students to ensure they are working tothe standards that the people using our services deserve

Essential

  • NMC Registered General Adult Nurse
  • Post Graduate degree or specialist cardiac qualification e.g. ENB 254, 129, 249 or equivalent
  • Advanced Physical Assessment qualification or willingness to undertake course (Level 7)
  • Caledonian Heart Failure qualification or willingness to undertake this within 12 months of commencing the post

Desirable

  • Community Nursing Qualification
  • Mentorship training

Knowledge and Experience

Essential

  • Proven Experience of working as a heart failure specialist nurse
  • Cardiovascular disease management/cardiac care at grade 6/7
  • Chronic disease management
  • Evidence based practice and conducting research
  • Managing a clinical caseload
  • Clear knowledge and understanding of medications used in the management of adults with heart failure, including titration
  • Knowledge of clinical/strategic issues relating to the management of heart failure in adults such as NICE and ESC guidelines, NSF, local and national guidelines and polic
