An exciting opportunity to join Surrey Downs Health & Care (SDH&C) HomeFirst Services, supporting the expansion of Urgent Community Response (UCR) and Virtual Ward services across acute and community settings, including the Mary Seacole Unit.

Working as part of a one-team multidisciplinary frailty service, you will deliver high-quality, person-centred care through Comprehensive Geriatric Assessment, supporting patients in their own homes, care homes, and hospital environments.

As a Physician Associate, you will assess and manage patients, initiate and interpret investigations, support treatment planning, and work closely with consultants, GPs, registrars, and the wider MDT. You will receive clinical supervision and peer support from experienced frailty clinicians while contributing to integrated, system-wide care delivery.

Main duties of the job

The post-holder will work across the virtual ward and Mary Seacole Unit, which are based at EGH. They will be part of the MDT that undertake holistic assessments and commence management plans. The PA will also be expected to contribute to the in-reach of the emergency department and SDEC to proactively identify and support home patients that could receive treatment in the community. The Mary Seacole Unit is an integrated unit that combines an acute frailty unit with a community rehabilitation unit. It is based within Epsom General Hospital. The unit supports patients living with frailty, acute medical illnesses and rehabilitation needs. Care is offered by an enhanced MDT, with a goal to support back to their place of residence as soon as possible. The post holder will be part of the medical team and will work closely with both junior and senior medical staff as well as all other members of the multi-disciplinary team (MDT) present on the ward. The appointee will share the responsibilities of ensuring ongoing medical care is facilitated including expediting investigations, making specialty referrals and ensuring management plans are carried out to completion. This will include ensuring discharge summaries are accurate and complete.
