We have an exciting opportunities for a Band 5 Nurses to work at Leatherhead PCN, within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference within your community, we would love to hear from you!

We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.

We have our own Learning & Development Department that have a range of different courses and development plans that give you the opportunity to grow your career.

Main duties of the job

PCN - Primary Care Network

It is a network formed from GP practice populations working in partnership with others to provide health and care to local communities.

What will PCNs do?

  • Provide ongoing care and support
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
  • Reduce risk of admission for patients
  • Support people in their homes, people in care homes and through in-reach in to hospitals where necessary
  • Work closely with the community hospitals and specialist teams

Each PCN has a community focused trimuvirate (group of three) of Lead GP / Community; Lead Clinician and Operational Manager. Specialist services and community hospitals work closely with the PCNs to ensure a whole system approach to working across Surrey Downs.
