Job summary

The post holder will act as a highly experienced expert practitioner, demonstrating advanced, holistic nursing assessment, management and evaluation of diabetes patients. The purpose of the post is to lead in the development of a comprehensive and integrated Diabetes service across the secondary care/community interface that has a consistent and sustainable impact on the care and experience of both patients and carers with a view to reducing unnecessary admissions with support and care in the community.

The Nurse Consultant will practice autonomously and will extend the role to provide expert clinical advice on the management of diabetes patients to peers, medical colleagues, doctors in training, Physicians Associates, Clinical Nurse Specialists, General Practitioners and District Nurses.

The post holder will foster a climate of research, innovation and clinical excellence through expert clinical practice and multidisciplinary working and will take the lead role in coordinating the inpatient diabetes service within the Trust and project lead the CGM/Freestyle Libre program.

We are seeking to appoint an enthusiastic and ambitious Lead Nurse who will undertake the professional leadership and management of the Diabetes CNS within the ESTH acute Trust which also includes specialist dietitians.

The post holder will have a proven track record as a Band 8 CNS in Diabetes within the community and secondary care
