Renal Patient Pathway Assistant (PPA) - Holiday dialysis and Renal POD
We are seeking a part time PPA working in our Renal POD coordinating incoming and outgoing holiday dialysis requests on behalf of our renal patients and also supporting the renal POD with general administrative tasks including the distribution of clinic letters, arranging patient appointments and dealing with incoming calls and emails in to the department.

We are seeking a part time PPA working in our Renal POD coordinating incoming and outgoing holiday dialysis requests on behalf of our renal patients and also supporting the renal POD.
The post-holder will work 22 hours. There can be some flexibility in how these hours are worked. You will assist in the provision of the highest quality patient care through dedicated administrative and clerical support and continuous improvement.
Based in the Renal POD at St Helier Hospital, G block the post holder will be the point of contact for all administrative and clerical issues relating to patients' pathway of care for holiday dialysis queries, tracking and supporting them from point of request to their holiday dialysis treatment.
The post holder will provide a patient focused service that supports clinical staff in the provision of efficient integrated pathways that enable the best use of resources and ensure that patients are fit, willing to be able to be seen and that they are treated in the right place, by the right person, at the right time.
The post holder will ensure high levels of patient care/confidentiality by assisting the Renal POD and by being an accessible customer focus and knowledgeable point of contact for all involved in the service.

Key Duties
- To coordinate all holiday dialysis requests (incoming and outgoing), including liaising with dialysis units to secure and confirm slot availability. To work closely with the HD transfers PPC and the individual dialysis units.
- Send all necessary paperwork and track progress of each holiday dialysis request received to ensure there are not any unnecessary delays to the patients holiday.
- Record all holiday dialysis requests electronically and use a spreadsheet to record your actions
- Advise patients of the holiday dialysis process and support patients with their holiday dialysis requests and keep them up to date with their applications.
- Follow up requests by ensuring all necessary paperwork is complete and required tests/results are completed where appropriate.
- Accurately record patient demographics and ensure all forms are completed electronically.
- Escalate any financial queries that may arise and seek advice from the ASM, SM or GM.
- Work as part of a clinical team assisting the Patient Pathway Coordinator to ensure that patients are admitted efficiently for outpatient appointments, pre-admission appointments, investigative treatments, diagnostic procedures and inpatient and day case operations within national guidelines and in accordance with the trusts Patients Access, Booking and Choice Policy. This includes: NHS Constitution, waiting time standards; Department of Health admission guidelines and booking rules. Ensure that patients are informed with reasonable notice, and that all relevant paper work is completed for given appointment. This includes ensuring that all elective attendances/admissions are promptly and accurately recorded on the appropriate PAS system(s) including E-referrals within one working day of receipt/decision to treat.
- To provide administrative support to consultant/clinical teams. Attend regular meetings with consultant teams, MDT teams, clinical staff, clinical directorates /departments and/or general managers as required for efficient processes.
- To ensure that the information on referral letters, outpatient status forms and waiting list cards is recorded accurately and comprehensively on the Trust PAS system(s). Check patient identity details including all demographic information during patient encounters. Under direction, amend PAS letters and patient leaflets to ensure that information distributed to patients is accurate and current.
- Respond to patient queries by answering the telephone and responding to emails via the general mailbox
