Job summary

Are you a Mental Health Nurse, Occupational Therapist or Social Worker, looking for an opportunity to work within community mental health services?

We have a range of opportunities for both experienced band 6 practitioners, or developmental posts for band 5 practitioners, to support the transition to band 6, and would welcome applications from existing Band 6's and newly qualified band 5's.

This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.

If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.

Main duties of the job

We are a multidisciplinary team consisting of a talented pool of both registered and non-registered mental health professionals.

We have moved away from the use of care programme approach (CPA) and now work with service users and carers under the "your team, your conversation, you plan approach", which is a strength based, intervention lead model of care, that supports the individual to address their own identified needs. We support the conversation through the use of a personal well-being and safety plan, as well the use of patient related outcome measures (dialogue +)

The duties will depend upon which role you are applying for:

For band 5 applicants the role will involve exposing you to the different pathways of care and include a individualised personal development plan and use of a competency framework supported through supervision, to ensure the successful applicant has the necessary skill and experience to undertake the role of a Specialist Band 6 Practitioner.

1. To undertake the full range of activities required to deliver an ongoing comprehensive mentalhealth service.2. To facilitate the delivery of Personalised Care and Support Planning in line with the Your Team your Conversation-Your Plan Policy and guidance.3. To record risk incidents and to collaboratively create and update Safety Assessments and Safety Plans4. To provide personalised interventions to support people with Mental Health difficulties5.
