Job summary

Reconnect is a health-based programme designed to bridge the transition from prison to community health services for vulnerable individuals, including both sentenced and remand prisoners.

As a care-after-custody service, RECONNECT aims to improve continuity of care for vulnerable people leaving prison by engaging with them prior to release and supporting their transition to community-based services, thereby safeguarding health gains made whilst in custody. RECONNECT services seek to enhance wellbeing, reduce health inequalities, and address health-related drivers of offending behaviour.

Although not a clinical intervention service, RECONNECT provides liaison, advocacy, signposting, and practical support to help individuals leaving prison engage with community health and support services.

Right to Work in the UK :This position is not eligible for Skilled Worker visa sponsorship. Applicants must already hold the legal right to work in the UK at the time of application, as the organisation is unable to support sponsorship for this post.

Main duties of the job

The post holder will be based within the Liaison and Diversion Service. The unique aspect of this role involves working with people leaving prison who are returning to the Wiltshire locality, supporting their engagement with relevant health services.

The role is dynamic and challenging. The aim of the service is to offer support to individuals as they are released from prison, ensuring that any health vulnerabilities are identified and addressed, and that they are supported to engage with appropriate pathways upon release. This helps them reintegrate into the community.

The post holder will engage with individuals prior to release from prison to assess their health vulnerabilities. A support plan will be formulated to provide contact upon release and throughout their journey towards reintegration. Contact will vary in frequency according to need, and the post holder will assist individuals in organising and attending health service appointments to reduce identified vulnerabilities. Liaising with partner agencies, including probation and health services, will be key to establishing support and stability within the community.
