Job summary
A vacancy has arisen for a Band 5 community mental health lead depot nurse (Key Worker) to join the later life Complex Intervention and Treatment Team (CITT) based in the Victoria Centre, Swindon.
Applicants will need to demonstrate knowledge in the areas of the delivery of mental health care in the community, with particular reference to the needs of Older People, together with an understanding of both the PWP and Care Management processes.
You will be required to provide key interventions for an allocated caseload of service users with substantial and complex mental health needs. Potential additional interventions could involve being responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as key worker for a defined group of service users. Support will also be offered to family and carers.
Applicants will be expected to demonstrate a high level of decision making skills and to work independently. Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes.
Main duties of the job
The CIT team has a cohort of clients who administer regular LAI (long acting injectable) medication. The clients attending this clinic are generally presenting long term stability in their mental health, are low risk and have little to no additional recovery needs. The main role of them engaging in the CIT team services is for their LAI medication to be administered.
There are times when this cohort of clients will have additional needs similar to the CIT team key worker role. This role may then require an acute increase in support. This support will entail brief interventions and when this work is completed for the clients to be handed back to only having the depot clinic.
The clinician will need to manage the day to day running of the depot clinic including follow up of missed depots, keeping paperwork updated in a timely manner, following through additional intervention needs that have been identified as summarized above.
The clinician will help with transferring clients to appropriate services when needed including stepping down to primary or third sector services and GP's. The clinician will help in the ongoing development of this new service provision development.
