Band 6 Memory Nurse - North Somerset

Job summary

As a Memory Service Nurse in North Somerset, you will participate as a full member of the multi-disciplinary Memory Service team.

You will be responsible for coordinating the service user's journey through their memory assessment through to diagnosis.

You will play a key role in preparing the service user and carers for the diagnostic process and in seeking consent from the service user regarding further assessment as appropriate.

You are expected to present your cases to the MDT each week.

You will be expected to work across several locations, including the service user's own home if indicated.

Main duties of the job

To provide initial specialist assessment for individuals experiencing problems with their memory and cognition.

Holding a caseload of service users whilst the process of their assessment and diagnosis progresses.

To liaise and participate with other members of the multidisciplinary team, family members, carers, Primary Care and other agencies involved with individual service users to formulate diagnoses.
