Job summary

An exciting opportunity has arisen for an experienced older adult clinician to join the BaNES Older Adult Community Mental Health Team, a well established, multi-disciplinary, integrated team.

The post is based in Bath and North East Somerset, a UNESCO World Heritage Site. It is a beautiful city, surrounded by amazing countryside locations, and offering an impressive variety of arts, culture and sports. Easily commutable from surrounding areas, including rail links to Bristol and beyond, there is staff parking available as well as access to pool cars, charge points for electric cares, and lock up facilities for those cycling to work.

Main duties of the job

You will be joining the team at a time when the key worker role (replacing care co-ordination) has been introduced. Your role will be to provide specific interventions to individuals across the workstreams, those individuals experiencing significant mental health needs, and those living with a dementia, in order to enhance their well being and maintain independence. This may involve working in a highly collaborative, holistic and solution focussed manner, delivering specific therapeutic interventions, supporting engagement with treatment, assessing and working with individuals in order to manage risks, assessing care and support needs, and supporting the introduction of appropriate care and support packages.

The post will offer the opportunity to develop and enhance supervisory skills, whilst providing supervision for both qualified and unqualified members of the team.

KEY RESULT AREAS: Clinical Practice 1) To be able to complete high quality, holistic assessment of an individuals need and risk following referral to mental health services. 2) To be responsible for a specific caseload of service users using the integrated CPA process and acting as care co-ordinator. 3) To establish with the service user and carer (where appropriate) an agreed well being plan, which includes evidence based interventions to address assessed needs, how outcomes will be measured and agreed timescale for input. 4) To commission such services as are appropriate to the individuals need, following funding agreement, or appropriate referrals. 5) To regularly evaluate the individual's plan with them and alter as required. 6) To use the principles and processes with the agreed ICPA policy and to act as care co-ordinator when appropriate. 7) To undertake specialist assessments, e.g. Cognitive testing/care act assessment. Record Keeping & Administration 1) To keep contemporaneous, chronological and accurate mental health records in accordance with Trust policy, professional guidelines and local team procedures. 2) To collect and input information and appropriate statistics for providing overall information about clinical activity. 3) To use time effectively and make appropriate use of resources. 4) To attend business meetings and participate in clinical discussion, presenting complex cases when appropriate. Clinical Liaison 1) To provide effective communication both within AWP and with Primary Care, Social Services and a range of other voluntary and statutory organisations. 2) To sensitively and appropriately communicate with colleagues, service users, carers and relatives. 3) To operate within the Multi Agency Policy on Safeguarding Adults and participate in the investigation process as appropriate. Continuing Professional Development and Practice 1) To maintain and develop professional knowledge skills and expertise, to ensure that practice reflects best practice and remain current and evidence based. 2) To act in accordance within the guidelines from your professional body. 3) To work to the policies and procedures of the Trust. 4) To ensure knowledge of and work to any local team policies and procedures. 5) To maintain up to date knowledge of relevant medicines, related medication management issues and act in accordance with Professional Body guidelines. 6) To take responsibility for post-registration education and practice requirements for continued professional development needs. 7) To ensure the provision and fully participate in the clinical and management supervision and appraisal process. 8) To actively participate in the Trusts policy of appraisal and staff development. 9) To maintain up to date knowledge and skills of appropriate IT systems.
