If you can answer yes to these questions then we want you to apply for our band 6 posts in Sarum CMHT. We have redesigned the model of care based on identified pathways. We are looking for clinicians to work within the psychotic pathway of our team.
Sarum CMHT is a multidisciplinary team who work well together. The role will include strong leadership and peer supervision. Despite having independent caseload there is an experienced and dedicated team approach.
Main duties of the job
The successful applicant will be responsible for the care co-ordination of a complex caseload of service users who have psychotic illnesses; this may include service users with co-existing substance misuse issues.
As well as overseeing medication regimens applicants will also be delivering therapeutic and psychologically informed interventions to service users or linking with other professionals to co-ordinate their interventions, this will all be done within the CPA process.
As part of the care coordination we expect a level of risk assessment and safety planning for service users under your care. With support systems in place for managing high levels of risk.
Main responsibilities:
Care co-ordination of a complex caseload of service users with psychotic illnesses.
Leading the CPA process.
Multi-agency working.
Risk management working.
Administration of medications.
Fulfilling the CMHT duty worker role on a rolling rota.
