The South Glos Recovery Team North are looking to recruit a qualified Specialist Recovery Practitioner to join our supportive and friendly team. The team is based in Thornbury near Bristol with a geographical area covering South Glos North including North Bristol, Filton, Southmead, Bradley Stoke, Little Stoke, Stoke Gifford, Patchway, Severn Beach and Thornbury plus many other numerous small Cotswold villages.
The team has a diverse group of experienced practitioners across Mental Health Nursing, Occupational Therapy and Social Work and are supported by a very experienced Consultant and Medics plus an Admin team.
All staff are actively encouraged to access additional training to enhance their role. Staff are currently undertaking Structured Clinical Management Training, DBT Training, Psychosis and Bipolar Training and Leadership training.
We offer a full Induction program plus several varied supervision structures alongside a mentor.
You must hold a Full Driving Licence and own a car for this role due to the geographically varied area.
Main duties of the job
This role is focussed on providing appropriate, effective interventions and treatments to people with severe and enduring mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services. The post holder will also provide clinical leadership and supervision to team members.
Always under the overarching framework of CPA, the post holder will be responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as care coordinator for a defined group of service users.
The role will also require undertaking and delivering specific health or social care assessments and interventions, according to care clusters, including identified service users on other caseloads. This maybe either on a one to one basis, or as part of a group activity.
