Job summary

The Dementia Enhanced Support Team provides psychosocial interventions at times of crisis as alternatives to admission to our later life dementia wards. The team supports individuals and family/carers within their own homes. Liaising with a variety of professionals, teams and services to facilitate care delivery.
The team sits within the Later Life Community Mental Health team and consist of one band 7 Senior Practitioner and two Band 6 Practitioners.

Main duties of the job
The Dementia Enhanced Support Team is a specialist dementia team aimed at providing a safe and supportive alternative to mental health /acute hospital admission for adults with a diagnosis of dementia who are experiencing some level of crisis or difficulty due to their illness.
The Team will provide a range of community-based health interventions for services users experiencing some level of crisis or difficulty due to their illness, working in an inclusive way to resolve the immediate difficulties and improve their situation.
Where appropriate, the post holder will work alongside other services to plan, deliver and review effective "wrap around" intensive interventions to manage risk and prevent hospital admission.
Please note that the South Glos DEST covers a large geographical area so applicants must have a valid driving licence and access to own transport. It is not possible to carry out the requirements of this role by using public transport
